An individual who is eligible for personal care service may receive
assistance with the tasks identified in this section when supported by
a documented need identified through a needs assessment and authorized
by a case manager. Reasons for the need to receive personal care services
because of an inability of a spouse or parent of a minor child to perform
the service must be documented in the needs assessment and the personal
care services must be authorized by a case manager.
The following activities and tasks, as defined on SFN
663, may be authorized to be performed by a personal care service
provider:
- Bathing
- Dressing/Undressing
- Feeding
- Toileting
- Continence/Incontinence
Care
- Transferring, Turning,
Positioning
- Mobility
- Meal Preparation
- Housework
- Laundry
- Shopping
- Medication Assistance
- Eye Care
- Nail Care
- Skin Care
- Hair Care
- Teeth, Mouth, Denture Care
- Money Management
- Communication
- Exercises
- Indwelling Bladder Catheter
Care
- Medical gases assistance
- Suppository assistance
- Temperature, Blood Pressure,
Pulse, Respiration Rate
- Prosthesis/Orthotics assistance
- Hoyer Lift/Mechanized Bath
Chairs assistance
